    Impact de l'indice de masse corporelle de l'homme sur les paramètres spermatiques et le pouvoir fécondant dans l’ouest de l’Algérie

    Résumé (en Français) : L’infertilité masculine est l’incapacité pour un homme de procrées du fait d’un défaut de son sperme. Les connaissances anatomophysiologiques sont indispensables pour la compréhension des différents mécanismes de l’infertilité masculine. De nombreux facteurs concernant le mode de vie et l’environnement sont susceptibles d’agir sur la fertilité masculine, en provoquant des altérations des paramètres spermatiques, quantitatives ou qualitatives. Nos objectifs étaient d’évaluer la relation entre l’Indice de Masse Corporelle (IMC) masculin et les paramètres spermatiques. Aussi, nous nous intéressons à l’étude de la probable corrélation entre IMC et la fragmentation de l’ADN spermatique. Le premier objectif de cette étude était de déterminer l'impact de l'indice de masse corporelle (IMC) sur les paramètres du sperme chez les hommes infertiles. L’échantillon est constitué de 446 hommes. La population étudiée a été divisée en quatre groupes en fonction de leur IMC (IMC insuffisant (<18,5 kg / m2) ; IMC normal (18,5-24,99 kg / m2), le surpoids (25-29,99 kg / m2) et l’obésité (IMC> 30,0 kg / m2). Les paramètres (PH, volume, concentration, nombre total de sperme, vitalité, morphologie et motilité) ont été déterminés dans les quatre groupes d’IMC. L'âge moyen était de 41,91 ± 6,39, la durée moyenne d'infertilité était de 4,92 ± 3,28, 351 (78,7%) avaient une infertilité primaire et 95 (21,3%) avaient une infertilité secondaire. L'IMC moyen était de 29,38 ± 4,85 et la plupart des patients étaient obèses. Cette étude a mis en évidence une association entre l'IMC et les paramètres du sperme (concentration de sperme, nombre total de spermatozoïdes, motilité et vitalité) et l'absence de corrélation entre le volume, la morphologie du sperme et l'IMC. Aucun lien n’a été noté entre l’IMC et la fragmentation de l’ADN (p=0.457). Le deuxième objectif de notre travail est d’étudier les différentes étiologies rencontrées dans l’infertilité masculine dans des cabinets privée d’urologies et génécologie obstétriques dans la wilaya de sidi bel abbés (Algérie). La tranche d’âge la plus touchée par l’infertilité masculine est celle de 31-40 ans. L’infertilité primaire est la plus fréquente avec 84% contre 16% d’infertilité secondaire. Les perturbations spermiologiques fréquemment rencontrées demeurent l’oligoasthénospermie avec 27,5% suivie l’azoospermie avec 20,6%. L’influence de l’activité professionnelle sur la fertilité de l’homme sont les professions dont les activités représentent la position (débout ou assise) prolongées et/ou en contact prolongé avec la chaleur (les militaires 21,3%, les commerçant 20%, les administrateurs 10%, les agriculteurs 7%). La majorité de nos patients 62,5% présente un IMC normal compris 20-24 et 34,4% en surpoids. La durée moyenne d’infertilité de nos patients 4,71±3,17 ans. La sensibilisation des hommes infertiles sur les facteurs de risque d’infertilité est recommandée afin d’augmenter la fertilité naturelle et de permettre une meilleure prise en charge de l’infertilité masculine avant tout traitement. Une investigation plus approfondie s’avère nécessaire devant l’élévation du taux d’infertilité d’origine idiopathique. Abstract (en Anglais) : Male infertility is the inability for a procreative man due to a defect in his sperm. Anatomophysiological knowledge is essential for understanding the different mechanisms of male infertility. Many lifestyle and environmental factors are likely to affect male fertility, causing alterations in spermatic, quantitative or qualitative parameters. Our objectives were to evaluate the relationship between the male Body Mass Index (BMI) and sperm parameters, as well as the effects of weight loss. Also, we are interested in the study of the probable correlation between IMC and nuclear integrity measured by the fragmentation of the spermatic DNA. The perspectives of this work are part of the global, personalized and multidisciplinary care of the two members of the infertile couple. Nutrition appears as a fundamental element, but also inseparable from other environmental factors. The primary objective of this study was to determine the impact of body mass index (BMI) on sperm parameters in infertile men. The sample consists of 446 men. The study population was divided into four groups based on their BMI (insufficient BMI (<18.5 kg / m2), normal BMI (18.5-24.99 kg / m2), overweight (25-29.99 kg / m2) and obesity (BMI> 30.0 kg / m2) The parameters (pH, volume, concentration, total sperm count, vitality, morphology and motility) were determined in the four groups of BMIs. Mean age was 41.91 ± 6.39 (Fig 1), mean infertility duration was 4.92 ± 3.28, 351 (78.7%) had primary infertility and 95 (21 , 3%) had secondary infertility, mean BMI was 29.38 ± 4.85 and most patients were obese.This study found an association between BMI and sperm parameters ( sperm, total sperm count, motility and vitality) and the lack of correlation between sperm volume, morphology and BMI.The second objective of our work is to study the various etiologies encountered in male infertility in private practices of obstetric urology and genecology in the wilaya of sidi bel abbés (Algeria). The age group most affected by male infertility is 31-40 years old. Primary infertility is the most common with 84% versus 16% secondary infertility. The frequently encountered spermiological disturbances remain oligoasthenospermia with 27.5% followed by azoospermia with 20.6%. The influence of professional activity on the fertility of the man are the professions whose activities represent the position (standing or sitting) prolonged and / or in contact prolonged with the heat (the military 21.3%, the tradesmen 20 %, administrators 10%, farmers 7%). The majority of our patients, 62.5%, have a normal BMI of 20-24 and 34.4% overweight. The average duration of infertility of our patients 4.71 ± 3.17 years. The sensitization of infertile men on the risk factors of infertility is recommended in order to increase the natural fertility and to allow a better management of the male infertility before any treatment.
