    Effet de l’agression environnementale sur la balance Th1/Th2 et le NADPH au cours du lymphome non-Hodgkinien : rôle des pesticides sur l’effet Warburg

    Résumé (Français et/ou Anglais) : Résumé Introduction. Le lymphome non-Hodgkinien (LNH) est un syndrome lymphoprolifératif. L’apparition du LNH a été corrélé positivement à une exposition aux pesticides dans de nombreuses études, en effet, les pesticides ont un rôle dans la lymphomagénèse et conditionnent l’évolution des LNH ainsi que le pronostic du patients. Le lien entre les pathologies cancéreuses et l'altération du métabolisme a déjà été suggéré comme une caractéristique commune des tissus cancéreux. Les cellules cancéreuses dépendent de la glycolyse pour produire de l'énergie, même en présence d'oxygène à des concentrations normales, c’est l’effet Warburg. Objectif. L'effet des pesticides sur le nicotinamide adénine dinucléotide phosphate hydrogène (NADPH) et l’altération du métabolisme « effet Warburg » ainsi que sa relation avec le rapport Th1/Th2 ont été évalué chez des patients souffrant de LNH. Matériels et méthodes. 100 patients nouvellement diagnostiqués avec un LNH agressif (53 hommes, 47 femmes) et 40 contrôles (23 hommes, 17 femmes), exposés ou non aux pesticides, ont été recrutés pour une étude cas-témoin, menée au niveau des Services d'Hématologie des Centres Hospitaliers et Universitaires de Tlemcen et de Sidi Bel-Abbès. Résultats. Les niveaux de NADPH ont été significativement élevés chez les patients par rapport aux contrôles; et chez les patients exposés aux pesticides (PE) par rapport aux non exposés (PNE) et aux contrôles (analyse unidirectionnelle de la variance, p=0,000). L'albumine, la glutathion peroxydase, la superoxyde dismutase, l'activité de catalase, et la capacité antioxydante totale (ORAC) ont été significativement diminués chez les patients en comparaison avec les contrôles, avec des niveaux de l'ORAC significativement diminués chez les PE par rapport aux PNE; cependant, les niveaux de malondialdéhyde ont été significativement augmentés chez les PE par rapport aux contrôles et aux PNE. Les taux de protéines carbonylées et de xanthine oxydase ont été significativement élevés chez les PE par rapport aux contrôles; alors qu'aucune différence significative n'a été observée entre les deux groupes de patients ou entre les PNE et les contrôles. Le rapport Th1/Th2 a été significativement diminué chez les patients par rapport aux contrôles; alors que le rapport neutrophiles sur lymphocytes a été significativement élevé (pour les deux comparaisons, P<0,001). En outre, le NADPH a été fortement associé au LNH (Odds ratio commun de Mantel-Haenszel = 5,55; intervalle de confiance 95% 2,22-13,88; P= 0,000). Aussi, les niveaux de NADPH ont été corrélés négativement au rapport Th1/Th2, soit chez les PE ou chez les PNE (respectivement, r=-0,498 [P=0,004] et r=-0,327 [p=0,006]). Conclusions. L'exposition aux pesticides a été fortement associée à l'altération du métabolisme « effet Warburg » et des taux de NADPH au cours du LNH. La relation entre le NADPH et le rapport Th1/Th2 devrait être au centre des nouvelles stratégies thérapeutiques contre la maladie. Mots clés. Lymphome non hodgkinien, pesticides, effet Warburg, NADPH, rapport Th1/Th2. Abstract : Introduction. Non-Hodgkin lymphoma (NHL) is a lymphoproliferative disorder. NHL appearance was positively correlated with exposure to pesticides in many studies. In fact, pesticides have a role in lymphomagenesis and condition the evolution of NHL and the prognosis of patients. The link between cancer diseases and altered metabolism has been suggested as a common feature of cancer tissue. Cancer cells depend on glycolysis for energy production, even in the presence of oxygen at normal concentrations, this is the Warburg effect. Objective. The effect of pesticides on nicotinamide adenine dinucleotide phosphate hydrogen (NADPH) and impaired metabolism "Warburg effect", including its relationship with Th1/Th2 ratio, in patients suffering from non-Hodgkin lymphoma (NHL) was investigated. Materials and methods. One hundred newly diagnosed patients with aggressive NHL (53 men, 47 women) and 40 healthy age-, sex-, and body mass index-matched controls (23 men, 17 women), exposed or not to pesticides, were recruited for a cross-sectional study conducted at the Clinical Hematology Departments of Tlemcen and Sidi Bel-Abbès University Medical Centers in the northwest of Algeria. Results. NADPH levels were significantly increased in patients compared with controls; and in exposed patients (EP) compared with those not exposed (NEP), and controls (one-way analysis of variance; P=0.000). Albumin, glutathione peroxidase, superoxide dismutase, catalase activity, and oxygen radical absorbance capacity levels were significantly decreased in patients compared with in the control group. Oxygen radical absorbance capacity levels were significantly decreased in EP compared with in NEP; however, malondialdehyde levels were significantly increased in EP when compared with controls and NEP. Protein carbonyl and xanthine oxidase levels were significantly increased in EP compared with controls; meanwhile, there were no significant differences between the two patient groups or between NEP and controls. The Th1/Th2 ratio was significantly decreased in patients when compared with controls; the neutrophil-to-lymphocyte ratio was significantly increased (for both comparisons, P<0.001). In addition, NADPH was strongly associated with NHL (Mantel–Haenszel common odds ratio estimate =5.55; 95% confidence interval, 2.22–13.88; P=0.000). Moreover, NADPH levels were significantly negatively related to the Th1/Th2 ratio, either in EP or in NEP (respectively, r=−0.498 [P=0.004] and r=−0.327 [P=0.006]). Conclusions. Pesticide exposure was strongly associated with the altered metabolism "Warburg effect" and NADPH rate during the NHL. The relationship between NADPH and Th1/Th2 ratio should focus on new therapeutic strategies for the disease. Keywords. Non-Hodgkin lymphoma, pesticides, Warburg effect, NADPH, Th1/Th2 ratio.
